Information you submit
Our contact, volunteer, employment, and enrollment forms send your information to Guided Grace FYS’s protected website database. Authorized staff use it to respond to inquiries, consider employment interest, and process enrollment. Enrollment information may include health, optional insurance details and card attachments, and emergency-contact details; the enrollment consent statements explain its intended use and sharing.
General contact messages should not contain medical or financial records. We do not collect payment-card details on these forms. Donations open PayPal’s separate payment service.
Questions, corrections, and access
Call (989) 401-8990 if you need help completing a form, want to correct a submission, or have a question about your information. Staff access to submissions requires an authorized sign-in.
